作为上述报名营员的家长或监护人，我同意我的孩子参加08 Tennisjoy精英少年网球夏令营。我明白参加这项运动以及相关的活动会有一定的受伤风险，我明白并接受这些风险的存在。我证明我的孩子在身体及精神方面都没有任何会妨碍或影响他/她参加本夏令营任何活动的问题。如果有任何的紧急事件或伤害事故发生并且联络不到上述申请表中的紧急情况联系人，我授权夏令营主办方为我的孩子进行紧急施救。因为我明白参加这项运动的风险和总体情况，所以在参与夏令营活动中如果我的孩子有人身意外伤害事故、个人财产物品丢失或被盗事件发生，我免除夏令营主办方及其工作人员、教练员、赞助商、合作伙伴、上级主管、其他营员及其家长、朋友的一切相关责任。我明白如果缺席活动、交通原因迟到、或因个人原因离开夏令营，费用不退。我同意遵守夏令营的一切规章制度。最后，我清楚并同意本夏令营的主办方有权将其在课堂教学活动中拍摄的任何影音资料用于发表及广告等用途。

Parent/Guardian Authorization and Disclaimer:

As parent or legal guardian of the above camper, I hereby give permission for my child to participate in the 08 Tennisjoy Junior Elite Summer Camp program. I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as other related activities incidental to my child’s participation, and am willing to assume these risks. I hereby certify that my child is fully capable of participating in this sport and that he/she is healthy and have no physical or mental disabilities or infirmities that would restrict full participation in this activity. In the case of accident or injury, where an emergency contact person cannot be reached, I grant Tennisjoy permission to obtain medical attention for my child. In addition to giving full consent for my child’s participation, I do hereby waive, release and hold harmless Tennisjoy, its officers, coaches, sponsors, partners, supervisors and representatives for bodily injury, property damage or theft of personal property that may occur while my child is enrolled in this program on or of Tennisjoy premises. This release applies individually and jointly to other campers, their friends or their family members. I understand that no refunds will be made for absences, transportation delays, withdrawals or dismissals. I agree to comply with all program rules and regulations. I further understand that Tennisjoy retains the rights to any photographs or video taken at the facility to be used for publicity or advertising.
